Secondary Athletic Accident Insurance

Mission CISD has purchased a limited benefit secondary accident insurance policy that covers the student - athletes while participating in UIL Athletic activities.   This limited benefit secondary accident insurance is a "secondary" coverage.  This plan is designed to help pay those expenses not covered / paid or payable by any other personal insurances. Therefore, you will be required to file first with your personal insurance covering your Mission CISD Student-Athlete. The Mission CISD limited benefit secondary accident insurance is a limited plan and will not pay 100% of the incurred medical bills or cover illnesses.  Your personal insurance is the primary carrier (including Medicaid), and the Mission CISD's limited benefit athletic insurance is the secondary carrier.

Any bills not paid by the parent / guardian's personal insurance carrier or the secondary school insurance will be the responsibility of the Parent / Guardian & the student - athlete. MISSION CISD OR ITS EMPLOYEES ARE NOT RESPONSIBLE FOR THE PAYMENT OF ANY SAID MEDICAL BILLS.  
 
Parent / Guardians must notify the coaches & athletic trainer prior to any non emergency athletic injury physician visit or the Mission CISD athletic insurance benefits may be forfeited.
 
A completed copy of the athletic insurance form and all physicians'  notes / instructions need to be turned into the AMJH Athletic Trainer after each physician's office visit please.

Risk Placement Services (RPS) Bollinger Specialty Group has contracted for fee discounts for services received from physicians and facilities participating in the First Health, Stratose, HealthSmart Preferred & PHCS networks.  Parent / Guardians are able to use any US based physician's office of their choice.  The hospital systems pictured below and most of their MD's are in the Bollinger provider network.
